Quadrant Biosciences Pooled Testing Information
If you have not opted out of the mandated weekly COVID-19 testing by providing proof of vaccination, you will need to provide proof of testing on a weekly basis. You may choose to participate in our weekly pooled testing to meet this obligation.
To do so, please log on to
https://app.clarifi-covid-19.com/ to register under User Roles as a Patient. You will be required to enter your information, including insurance information. If you do not register prior to scheduling your test, you will not be able to participate in the testing and you will be responsible for receiving/submitting a test on your own.
